Overview

TNI offers individuals and families a broad range of services that meet both short- and long-term needs to help kids and their parents reach their goals.

Here’s how TNI is helping residents today:

Family Support

TNI area residents are eligible for Family Support services from THT’s community care team. Community health workers build trusted relationships with residents and provide targeted, one-on-one support to increase family health and stability. Support can include, but is not limited to, addressing healthcare needs, securing health and other forms of insurance, utility and rental arrears, school supplies, internet access, transportation, and more. For more information, contact Miss Snow at 609.256.4555 x400 or tni@trentonhealthteam.org. Family Support is available in addition to Family Connects for those who live in the TNI area.

This program compliments the goals of another program Capital Health supports, Family Connects, which provides a free in-home nurse visit to all families giving birth at Capital Health – Hopewell and living in Mercer County, regardless of income or background.  For more information visit the Family Connects website. Increasing Child Lead Screening

We are working with social services organizations, healthcare providers, and government agencies to improve the early detection of lead and empower residents to take action to prevent lead poisoning.
